I suspect it may be Beavis Audio's "everything fuzz" of which there are a few pics. Since that has never been a commercial product, the Infinitphase would likely be first... 19 knobs although only 1 switch...

But to answer your question it is actually called a HEXE Lunochod

Creates wacky waveforms for expression inputs on other HEXE units like their Radar Phase, which seems dang groovy from the clips I've seen.
