If I have an amp with an 8ohm speaker, is there a way to add an extension cab and have both the original speaker and cab working at the same time?

More information is needed. What amp, what cab?

It is a custom made 18w amp. I would use an extension cab that was either 1x12 or 2x10. I'd only use it for bigger stages where I'd want a fuller sound on stage. I might get speakers that are more sensitive/louder too.
Right now the amp has a lineout and I am assuming I'd have to get it changed to a ext. speaker out.

Depends on if the amp can handle a 4 ohm load (or 5.33 ohm load if you use a 16 ohm ext cab). Most well built amps should have no problem doing so. I wired in an extra jack off the speaker in my Rivera Chubster and mounted it off the cross brace on the back on the amp with an L bracket to do exactly the same thing. Bob
