Alot of really good T-style builder out there that have already been mentioned. My personal favorite though is Hahn Guitars. I have two of Chihoe Hahn's masterpieces: one is a vintage style (ash body maple/maple neck, Lollar pups) blackguard, which just might be my favorite guitar ever, and the other is a modern rocker (alder, rosewood/maple neck, fralin pups). Check him out at either hahnguitars.com or destroyallguitars.com. Not to mention, Chihoe is wicked cool to deal with.

Gene Baker makes a Tele type guitar, the Metal XT, if my memory serves me.
I saw Grosh mentioned, how about Melancon, Anderson, and Ron Kirn.
Oh, and Scott Heatley can make one for you also.
Also, got to throw out some love for Fender. I hear they make a pretty good Tele.
Then, you've got the telepauls, like the Gustavvson Bluesmaster, Ian Anderson Standard, Huber Dolphin, etc.

I would suggest to anybody considering a "T" type guitar that they contact Ron Kirn. After you speak with him, you will know why. Meanwhile, check out his website and read the text to get a taste of Ron's general philosophy.
Ron is exceptionally great to deal with, and my Kirn "Basic T" is superb.

$2,500 w all the good stuff (fralins, callaham etc)
no one's mentioned rick kelly yet!!!!
here's my bowery pine tele.
Pine from a building they were taking down on the Bowery in NYC. It's about 125 years old. 80 Year old maple neck. Don Mare pups and a nice beautiful fat neck.
All for a whopping $1400. The only downside is the wait- up to a year or more, though i got mine in 6 month by incessantly bugging Rick and going into his shop weekly.

How do you guys specify the necks when you are getting one of these made? Have you played so many fender necks that you can quote dimensions or something? Or do you say "fairly chunky for a guy with average size hands" or something along those lines?
For me an awful lot of whether I like a guitar is the feel of the neck but I have no idea how you specify that to a builder and can't really see the point of having one made if the neck is not pretty much guaranteed to blow me away.

Descriptions, comparisons to known quantities (commonly available, consistent guitars), a little bit of faith...
Some people know the numbers that work for them. Numbers are easy to come by with the purchase of an inexpensive caliper and measuring your guitar you like.
The biggest thing is choosing a luthier who is a good fit to you. Describe your playing style, what you like, what you dislike, and take a leap of faith.
