Can a strat's 5 way be wired like this...

I normally do not use the middle single alone, or the middle/bridge setting.

I'd like to replace those with:

All pickups on, like a guitar with a blender pot
Neck+Bridge

Is it possible to get those settings with a regular 5 way switch?

Yes. You can either get a push/pull pot or do what I did. I added a toggle switch wired to my neck pickup(ala Gilmour) that I can activate in any position.

Check out the Deaf-Eddie Fat-o-caster switch (http://www.deaf-eddie.net/buyit.html). Replaces your neck tone pot. Gives you lots of additional pickup combinations while still retaining your 5-way selector.

Can I do it without a toggle or blend pot? I'd liek to replace the #3 and #4 positions with those I mentioned.
