Identify this pedal

Hey all --
I've been reading for a while and finally registered.

I was wondering if anyone can identify this pedal. I saw it used by Jakob Dylan between his electric and amp and haven't been able to place it.

I can't imagine it is anything else. Have a look at the photo below, where you can make out the two holes on the left side of the case near the top - an unusual feature, to be sure.

Indeed it's a /13. The config of the pots & switches is unique.
