For me, a Tone Factor Nebula (Phase 45). It was one of my first pedals I got when I started playing again about 8-9 years ago.

Zvex Fuzz Factory - it's been more or less constant for the past 2-3 years, which is sadly a long time for me.

While not on my board, the pedal I still "use" once every few years is the first pedal I ever got...a mid-80's Yamaha distortion, like this:

I had a MutronIII that I'd had since 1978 (36 years), but sold it last year and got a Q-tron+
My current oldest pedal is the Rotosphere that I bought in '98 (17 years)

Good ol' Cry Baby. probably 12 yrs on my board now. I messed with it a bit, manually adjusted the sweep so it doesn't go as high and also switched out the fasel inductor for a better one

In general, a wah. I've never had a board without a wah and would sooner give up guitar all together than give up having a wah on my board.

More specific, I've never had a board without a Phase 90 on it. Even though I don't use it too often. Come to think of it it's my least used pedal on my board yet I've never been without one.

I still have my green russian big muff and dunlop crybaby which were both purchased new in the mid 1990s. Around 20 years now! The crybaby is currently broken but the muff is going strong!

I think I've had my Memory Lane on my board since '06. I've got some other stuff I bought in that time period but not actually on my board.
I think I've owned an Analogman TS9 silver the longest (bought in '05). Didn't really use pedals before that (started playing in '90).

Boss TU-2. I've had it since 2003 and it's been the only pedal that has been on every iteration of my board. Hasn't failed me yet.

Jetter Gain Stage Gold – 6 years of consistent, on-the-board weekly/daily usage.

Crybaby Wah – Have owned for at least 20, was a regular for about 15 of those, but hasn’t seen any play in the last 3-5 years, or at least I can’t remember when the last time I plugged it in was in that time period.

Boss TU2, for at least 15 years...

Then it would be my BB Preamp, bought it new in 2006, never left me since then!

It's not on my board but I'll use my SD-1 i got about 8 years ago at home. The longest pedal I've used on my board is probably the Nova Repeater. I've been using it for about 3 years.
